There are two different ways to submit your team roster and site information:

Method 1: Click on Team Roster and Site Information Form below. Fill in the information, click on the Print button if you want a hard copy, and then just click on the Submit button, and send the email.

Method 2: Click on Team Roster and Site Information Form below. Click on the Print button, fill out the information by hand, and mail or hand deliver it to your coordinator before the deadline.
